Activity Permission Forms for Term 3 2020

Dear Families,

We have been advised that at this stage, some Wilston State School extra-curricular activities will be returning in Term 3 Week 2. This is as per the Stage 3 restrictions commencing on 10th July 2020. However, if there are any changes, we will communicate with families.

With activities starting back, this means that you will need to provide Wilston Kids Care with an Activity Permission Form. All previous permission forms pre Covid-19 were cancelled and will need to be completed again.

To make sure we get the correct forms and process them efficiently, we thought we would go through the common questions we get asked regarding Activity Permission Forms. We hope this is helpful for you.

What activities require permission forms?

All Wilston State School based extra-curricular activities that run out of school hours. For example, swimming, jujitsu, tennis, coding, drama and many more.

Why do I need to complete the form and not just tell you in person?

Under the National Law and Regulations to which our Service is governed, all children attending an excursion, or an extra-curricular activity must have a Permission Form giving written consent by a Parent/Guardian that they can participate. This is a rule to ensure safety and account for children leaving the Wilston Kids Care premises or moving to another location.

Without parental written permission as required under the National Law and Regulations, the child is unable to attend the activity.

How do I complete the Activity permission form?

This form is different from the Vacation Care activity permission forms.

What end date do I use?

If your child’s activity is only running for several weeks, for example when external vendors such as Qld Reds Rugby attend, you would document the start date your child is eligible to participate, and the end date to which your child finishes the activity.

If your child is participating in a permanent yearly activity such as tennis, you can put the start date as their first day of attendance, and the end date as the end of Term 4 2020. This way, the activity form stays in place all year, and only needs to be amended if there are any changes.

How often do I need to complete the form?

The forms need to be completed each calendar year preferably before Term 1 starts and whenever there are any changes to their activity. For example, your child moves up a class in swimming therefore their lesson times change.

Can I put all children on the one forms?

No. We do require an individual form for each child. However, we have made these forms as quick and easy to use as possible whilst still within the parameters of our Regulatory requirements.

How do I cancel an activity?

To advise Wilston Kids Care that your child is no longer attending an extra-curricular activity, all you need to do is send an email to the Admin team on wkc.admin@wilstonpandc.org.au. Please me mindful this does not advise the external vendors of the change. For example, to cancel a swimming lesson, you will need to contact Wilston Crocs and Wilston Kids Care.
